What Acetyl L-Carnitine Is, and How It Differs from Plain L-Carnitine

Carnitine ferries fatty acids into the mitochondria, where they are burned for energy. Your body makes it from lysine and methionine, and you get more from red meat and dairy, so a mixed diet rarely leaves anyone short.

The acetyl form is the one that crosses the blood-brain barrier readily, which is why it is studied for memory and mental fatigue rather than for muscles. Plain L-carnitine is the one studied for exercise and heart conditions. They are not interchangeable, and if what you want is the muscle and exercise version, buy that one instead — we stock it, and it costs less.

The Dose Question, and What This Bottle Actually Gives You

This is the part the category tends to leave out, so here is the arithmetic in full. Fat Loss: The Honest Answer

Carnitine’s job in the cell is to move fat into the furnace, and the marketing takes a short step from there to burning fat. The step does not hold. Transport is not the limiting factor in how much fat you burn — the energy deficit is — and in people who are not deficient, supplementing carnitine does not meaningfully change body fat. Where small effects appear, they are small, slow and inconsistent.

Who Should Not Take Acetyl L-Carnitine

Do not take it if you have an underactive thyroid. Carnitine interferes with thyroid hormone activity at the tissue level, which is the opposite of what treatment is trying to achieve. If you take levothyroxine, this is not your supplement.

Do not take it if you have epilepsy or any history of seizures. Carnitine has been associated with an increase in the frequency or severity of seizures in people who already have them.

Leave it alone in pregnancy and breastfeeding, where there is not enough safety information to justify it, and speak to a doctor first if you have kidney disease or are on dialysis. It is for adults.

Medicines to Bring to the Counter First

  • Warfarin, acenocoumarol or another blood thinner — carnitine can strengthen their effect, which means more bruising and bleeding. If you take one of these, do not start without telling whoever monitors your INR.
  • Levothyroxine or any thyroid medicine — see above; this is a reason not to take it rather than a reason to adjust.
  • Taxane chemotherapy, or nerve pain after it — acetyl L-carnitine has been found to make that particular nerve pain worse, not better, which is the reverse of what people take it for.
  • Any seizure medicine — the seizure caution applies whether or not the epilepsy is controlled.

Side Effects, Including the One Nobody Warns You About

A fishy smell to the breath, sweat or urine is the characteristic one. It is harmless, it is dose-related, and it is the reason a number of people stop. Nobody mentions it at the point of sale, so we will.

Otherwise: stomach upset, nausea, dry mouth, headache and restlessness. Taking it with food helps the stomach, and keeping the last dose away from bedtime helps the restlessness.

How to Take It

Split the day’s capsules rather than taking them together, with the last one by mid-afternoon. Take them with food if your stomach objects.
