Aloe vera juice supplies the liquid pressed from the aloe leaf, taken by the small glassful for digestive comfort and regularity, at 25ml once a day from a 500ml bottle.

Aloe is one of the very few plants where how it is processed matters more than how much of it you take. The same leaf yields two quite different things, one gentle and one a stimulant laxative that regulators have acted on. Which one is in a bottle depends entirely on the manufacturing, and that is the first thing to establish about any aloe drink.

Inner Gel, Whole Leaf, and Why the Difference Matters

The clear gel in the middle of the leaf is the gentle part. It is mostly water with polysaccharides, and it is what people mean when they talk about aloe soothing the gut.

Between that gel and the green rind sits a thin yellow layer of latex. It contains aloin, an anthraquinone that acts as a stimulant laxative. A whole-leaf product includes this layer unless the aloin is deliberately taken out.

Removing it is a specific step, and filtering is not it. Filtering takes out pulp. Aloin is removed by passing the juice through activated charcoal, which is called decolourisation. A responsible whole-leaf aloe drink is decolourised and tested, and the accepted threshold for a drinkable product is no more than ten parts per million of aloin. A 2023 review found drinks meeting that threshold were not genotoxic.

We check this before we stock an aloe drink, and we will tell you the answer for this one if you ask. If a bottle anywhere does not say decolourised, purified, or aloin-free, that is a fair question to put to whoever is selling it.

What the Evidence Supports

Short-term use of oral aloe gel, up to about six weeks, is regarded as safe. Beyond that the picture thins out quickly, and the honest summary is narrower than the category suggests.

Digestion and regularity. This is the traditional use and the one people report. Where the effect is dramatic rather than gentle, that usually means aloin, not gel.

Blood sugar and weight. Oral aloe has shown modest reductions in both in people with diabetes. Modest is the word, and it is also a caution rather than only a benefit, which is below.

What is not supported. There is not enough evidence for irritable bowel syndrome, ulcerative colitis, eczema or diabetic foot ulcers. Applied to the skin is a different matter, where aloe genuinely helps burns and several skin conditions, but that is a gel on the skin and not a drink.

The Liver, Plainly

Aloe vera juice is not a liver supplement, and this is the point on which we depart from most of the category. Oral whole-leaf aloe extract taken over periods from three weeks to several years has been linked to cases of acute hepatitis in the published literature. Cases, not a common event, and mostly with whole-leaf preparations rather than gel.

That is the opposite of a liver benefit, and anyone selling aloe as liver support is stating something the evidence points the other way on. If you already have liver disease, or you take anything hard on the liver, speak to our pharmacist before starting this. If you are taking it and your eyes or skin yellow, your urine darkens, or you become unusually tired and off your food, stop and get a liver test.

Before You Start, Check These

Speak to our pharmacist first if any of these apply:

  • Pregnancy or breastfeeding — avoid it. Taken by mouth, aloe in gel, latex and whole-leaf forms is all regarded as possibly unsafe in pregnancy and while breastfeeding.
  • Digoxin or a diuretic — a laxative effect loses potassium, and low potassium makes digoxin dangerous. This is the most serious interaction on the list.
  • Diabetes medicine or insulin — aloe can lower blood sugar modestly, which adds to what your medicine is already doing.
  • Liver disease — see above.
  • Kidney disease — through the same potassium and fluid route.
  • Inflammatory bowel disease, or ongoing abdominal pain of unknown cause — a laxative effect is the wrong thing to add before somebody knows what is happening.
  • Children — this is for adults.

How to Take It

Shake the bottle well, because aloe settles. Take 25ml once a day, either straight or stirred into water, fruit juice or a smoothie. Take it with or after food if it does not agree with you on its own.

A 500ml bottle at 25ml a day lasts twenty days. Give it a fortnight before judging whether it suits you, and keep it to short courses rather than taking it indefinitely, which is where the longer-term reports come from.

Stop if you get cramping or watery stools. That is not the product working harder, it is a laxative effect, and it means the dose or the product is wrong for you.

Side Effects

The common ones are abdominal cramping, loose stools and wind. Taken at the stated dose, most people get none of these.

Stop and speak to us about persistent diarrhoea, muscle weakness or an irregular heartbeat, which can follow potassium loss, and about yellowing of the skin or eyes.

What Is in the Bottle

Aloe vera juice, with ascorbic acid as an antioxidant and potassium sorbate as a preservative. That is the whole list, which is a good sign in this category, where sugar and flavourings are common.

Storing It

Keep it cool and out of direct sunlight before opening. Once opened, keep it in the fridge and use it within the period stated on the label, because there is very little preservative in it. Shake before every dose. Keep out of reach of children.

Is it a detox?

No. Your liver and kidneys do that continuously and no drink assists them. What aloe may do is affect how easily your bowels move, which is a real effect and a different thing from cleansing.

How much should I take and for how long?

What if it gives me cramps or diarrhoea?

Stop taking it. That is a laxative effect rather than a sign that it is working, and it is worth telling us, because a forceful effect from an aloe drink raises the question of what is in it.

Does it help with weight loss?

Only modestly, in studies of people with diabetes, and not in a way that replaces the things that actually change weight. Anything dramatic you feel from an aloe drink is your bowel, not fat loss.

Is aloe on the skin the same thing?
