Zeptide 7.5mg – Tirzepatide Step-Up Dose
Zeptide 7.5mg is tirzepatide at a step-up dose, a once-weekly 0.5ml prefilled syringe used to help control blood sugar in adults with type 2 diabetes and, where a prescriber judges it right, for weight management. It follows at least four weeks at 5mg, and it is a prescription-only medicine.

About Zeptide
Zeptide is tirzepatide made by Beximco Pharmaceuticals in Bangladesh. Each box holds one prefilled syringe of 0.5ml containing a single weekly dose. It contains the same active medicine as Mounjaro, from a different manufacturer; the strengths correspond, the device and packaging do not. Beximco makes Zeptide in three strengths only, 2.5mg, 5mg and 7.5mg.
How it works. Tirzepatide is a dual agonist: it activates the receptors for two gut hormones, GIP and GLP-1. That prompts the pancreas to release insulin when blood sugar rises, lowers glucagon, slows how quickly the stomach empties and reduces hunger through the brain. Semaglutide, in Ozempic and Reglutide, works on GLP-1 only, so the two medicines are not interchangeable and their milligram doses cannot be compared.
What it is used for. In the United Kingdom and the European Union, tirzepatide is licensed to help control blood sugar in adults with type 2 diabetes, and for weight management in adults whose body mass index is thirty or more, or twenty-seven or more with a weight-related condition, alongside diet and activity. Your prescriber decides whether either use fits you.
Its effect depends on continuing it. In a trial in which people who had lost weight on tirzepatide were switched to a dummy injection, most regained much of the weight within a year, so a prescription for weight is a long-term plan rather than a course.
Zeptide 7.5mg in the Treatment Schedule
Zeptide 7.5mg is not a starting dose. For a first injection it is three times what the body should meet first, and it is likely to cause severe sickness. It is the third step in the sequence:
- Start: 2.5mg once weekly for four weeks, which prepares the gut and is not a treatment dose
- Then: 5mg once weekly, the first treatment dose and often the lasting one
- If needed: 7.5mg, then 10mg, 12.5mg and 15mg, each step no sooner than every four weeks
- Ceiling: 15mg is the maximum weekly dose
One syringe is one weekly dose, so four weeks at any strength means four boxes. Buying the four together keeps the step unbroken, and all of them belong in the refrigerator.
A prescriber moves you to 7.5mg only when 5mg is not doing enough, and only after at least four weeks at 5mg. In the United Kingdom and the European Union it is usually a stage on the way to the 10mg maintenance dose, although some people stay on it when it works. Either way, give it at least four weeks before the next change.
Expect the side effects to return for a week or two after the increase, then settle. The contraceptive-pill caution applies again after every increase.
Switching in from another medicine is not like for like. From semaglutide, such as Ozempic or Reglutide, your prescriber will usually start tirzepatide low and step up. From another tirzepatide brand the milligram dose carries across, but after a break of more than one or two weeks the safer restart is often lower.
Storing Zeptide
Zeptide belongs in a refrigerator between two and eight degrees Celsius, kept in its box away from light and well away from the freezer section. It must not be frozen; a syringe that has frozen is thrown away, even once thawed. If a refrigerator is not available, for example while travelling, an unopened syringe may be kept at up to thirty degrees for no more than 21 days. For anything else, follow the carton.
A syringe spoiled by heat or cold looks completely normal, which is why storage history matters more than appearance. We keep every GLP-1 medicine in monitored cold storage between two and eight degrees and dispatch it cold in insulated packaging. When yours arrives, confirm the parcel is still cold, the liquid clear and colourless to slightly yellow with nothing floating in it, and the batch number and expiry the same on box and syringe. If anything is wrong, do not inject it; ring us and we will replace it.
Using Zeptide Each Week
- Pick an injection day and stick to it; food makes no difference. To change the day, leave at least three days apart between two doses
- Inject into the fatty layer of the abdomen, thigh or back of the upper arm, never into a vein or muscle, moving to a new spot each week
- Each syringe is single-use. Inject the full contents, never reuse it and never share it, even with family
- Missed a dose? Inject within four days of the usual day. After four days, skip it and carry on from your normal day. Never double up
- Used syringes go into a sharps container, never the bin, and a full container can be returned to us

When Your Dose Goes Beyond Zeptide 7.5mg
Zeptide stops at 7.5mg, and tirzepatide does not. If your prescriber decides you need 10mg or more, that means changing to another tirzepatide brand at that strength. Because the molecule is the same, the milligram dose carries across, but the device, the storage allowance and the look of the pack will be different.
Plan the change before your last 7.5mg syringe, so there is no gap in treatment. Ask us which brands we hold at the next strength, and read the new carton before the first injection rather than assuming it works like Zeptide.
Zeptide Alongside Other Medicines
If you use insulin or a sulphonylurea such as gliclazide or glibenclamide, the dose of that medicine usually has to come down when tirzepatide begins, or your blood sugar may drop too far. Only your prescriber should make that change. Tirzepatide is not a substitute for insulin.
Oral contraceptives can become less reliable for four weeks after starting and for four weeks after each increase, because tirzepatide slows stomach emptying and alters how tablets are absorbed. Use a barrier method as well, or a non-oral method, during those weeks. Keep your prescriber informed about every other medicine, and warn any surgeon, doctor or dentist before an anaesthetic or sedation.
Zeptide Side Effects and Warning Signs
Digestive effects are common, mainly nausea, vomiting, diarrhoea, constipation, heartburn and a smaller appetite, and they are most noticeable early on and after each increase. Small, plain meals and stopping as soon as you feel full help most. Replace fluids if you are vomiting or have diarrhoea, because dehydration can damage the kidneys.
Get medical help the same day for severe abdominal pain that persists, especially pain going through to your back, which can mean pancreatitis, and for swelling of the face or throat or difficulty breathing. Report soon: pain under the right ribs, yellow eyes or pale stools, which may signal gallstones; a change in your vision if you have diabetes; and a lump in the neck, difficulty swallowing or a hoarse voice that lasts.
Who Should Not Use Zeptide
People with a personal or family history of medullary thyroid carcinoma, or with multiple endocrine neoplasia type 2, must not use it. That is the specific thyroid contraindication, not thyroid problems in general.
Nor is it for people with type 1 diabetes, anyone under eighteen, anyone allergic to tirzepatide, or anyone pregnant, planning pregnancy or breastfeeding; your prescriber will usually stop it before a planned pregnancy. Raise with them first any history of pancreatitis, gallbladder problems, severe stomach disorders such as gastroparesis, kidney disease or diabetic eye disease.

No. Beximco makes Zeptide in 2.5mg, 5mg and 7.5mg only. If your prescriber moves you to 10mg or more, that means another tirzepatide brand at that strength. Ask us before your last 7.5mg syringe so there is no gap.
I am on Ozempic. Can I switch straight to Zeptide 7.5mg?
Not without your prescriber. Semaglutide and tirzepatide doses do not convert, and a switch usually starts tirzepatide lower before stepping up.
How should Zeptide be stored?
In a refrigerator at two to eight degrees, in its box, never frozen. An unopened syringe may be kept at up to thirty degrees for no more than 21 days. A syringe that has frozen must be thrown away.
What should I check when Zeptide is delivered?
That the parcel is still cold, the liquid clear and colourless to slightly yellow, and the batch number and expiry the same on box and syringe. A spoiled syringe looks completely normal, so call us if the parcel arrived warm.
