Tirzide 15mg is tirzepatide at the maximum dose, a once-weekly prefilled syringe used to help control blood sugar in adults with type 2 diabetes and, where a prescriber judges it right, for weight management. It follows at least four weeks at 12.5mg, and it is a prescription-only medicine.

If you do not have one, our doctor can assess you and issue one where it is appropriate — a proper assessment rather than a formality, which means the answer is sometimes no. What Tirzide Is

Tirzide is tirzepatide made by Everest Pharmaceuticals in Bangladesh, supplied as a single-dose prefilled syringe. It is the same active medicine as Mounjaro, from a different manufacturer, and each syringe holds one weekly dose in half a millilitre. The batch number and expiry date are printed on every carton.

Tirzepatide acts on two hormone receptors, GIP and GLP-1, which makes it a dual agonist. Together they increase insulin release when blood sugar is raised, reduce the glucagon that pushes it up, slow the emptying of the stomach and act on the parts of the brain that regulate appetite. Semaglutide, the medicine in Ozempic, acts on GLP-1 alone. The two are not interchangeable, and a dose of one tells you nothing about the right dose of the other.

Its licensed uses, in the United Kingdom and the European Union, are two. It helps control blood sugar in adults with type 2 diabetes, and it is used for weight management in adults with a body mass index of thirty or more, or twenty-seven or more with a weight-related condition such as raised blood pressure or sleep apnoea, together with diet and activity. Which of these, if either, applies to you is your prescriber’s decision.

The benefit lasts only while the treatment does. When people who had lost weight on tirzepatide were moved to a dummy injection in a trial, most regained a large part of it within a year. A weight prescription is planned as long-term treatment, with a plan for stopping agreed at the start.

Tirzide 15mg in the Dose Schedule

Tirzide 15mg is the top of the schedule, reached only after at least four weeks at 12.5mg: at least twenty weeks of stepping up. It is six times the starting dose and is never a first dose. The sequence:

  • 2.5mg once weekly for four weeks — the starting dose, which is not a treatment dose
  • 5mg once weekly — the first treatment dose, and for many people the dose they stay on
  • 7.5mg, 10mg, 12.5mg, 15mg — only if more effect is needed, each increase no sooner than every four weeks
  • 15mg is the maximum — never exceeded

One syringe is one weekly dose, so each four-week step uses four syringes. Ordering the four together keeps the step unbroken; all four go straight into the refrigerator.

15mg is the highest recognised dose, and it is not exceeded. Two syringes are never taken in one week to reach a larger dose, and a missed dose is not made up by doubling. In SURMOUNT-1, adults without diabetes on 15mg lost about 20.9 percent of their body weight on average over 72 weeks, against about 3 percent on a dummy injection; in SURPASS-2, adults with type 2 diabetes on 15mg lowered HbA1c by about 2.3 percentage points on average. These figures come from the original manufacturer’s trials and are averages, not a promise.

The difference between 10mg and 15mg in those trials was modest, and side effects were more frequent at the higher dose. That is why many prescribers stop at 5mg or 10mg, and why 15mg is used only when the lower doses are not enough.

After a break in treatment, do not restart at 15mg. If more than one or two weekly doses have been missed, speak to your prescriber or our pharmacist first; a lower restart is usually safer.

Keeping Tirzide Cold

Store Tirzide in a refrigerator between two and eight degrees Celsius, in its carton to keep out light, and away from the freezer compartment and the cooling plate. It must not be frozen, and a syringe that has frozen is discarded even after it thaws. The manufacturer’s leaflet allows an unopened syringe to be kept below thirty degrees for up to 21 days, which helps when travelling; after that it is discarded if unused. When in doubt, follow the carton.

Heat and freezing leave no visible mark, and a damaged syringe looks completely normal. That is the reason we hold every GLP-1 medicine in monitored cold storage kept between two and eight degrees and send it cold in insulated packaging. On arrival, check that the parcel is still cold, that the solution is clear and colourless to slightly yellow without particles, and that the batch number and expiry on the carton match the syringe. If anything is wrong, keep it aside and call us for a replacement.

How to Inject Tirzide

  1. Choose one day of the week and keep to it, at any time of day, with or without a meal. The day can be moved if doses stay at least three days apart
  2. Inject under the skin of the stomach, thigh or upper arm, never into a vein or a muscle, and use a different spot each week
  3. A prefilled syringe is used once and thrown away. Give the whole dose, never reuse it, and never share it with anyone
  4. A missed dose can be taken within four days of the usual day. Later than that, leave it out and go back to your normal day. Two doses are never taken together
  5. Put used syringes in a sharps container, not household rubbish, and bring the full container back to us

For your first injection, our pharmacist can walk you through it at the pharmacy or on a video call. Let the syringe stand out of the refrigerator for about half an hour first; a cold injection stings more.

Tirzide and Your Other Medicines

Insulin and sulphonylureas such as glibenclamide or gliclazide usually need a lower dose once tirzepatide starts, because the combination can take blood sugar too low. Your prescriber makes that adjustment. Tirzepatide is not a substitute for insulin and is never used in place of it.

The contraceptive pill may work less well for four weeks after the first dose and for four weeks after every increase, because tirzepatide slows stomach emptying and so changes how tablets are absorbed. Add a barrier method, or use a non-oral method, through those weeks. Tell your prescriber about everything else you take, and tell any doctor or dentist before a procedure with an anaesthetic or sedation.

Side Effects, and When to Get Help

Stomach and bowel effects are the usual ones: nausea, vomiting, diarrhoea, constipation, indigestion, belching and a reduced appetite, strongest in the first weeks and after each increase. Eating smaller, less fatty meals and stopping at the first sign of fullness helps. Keep drinking if you have vomiting or diarrhoea, because dehydration can harm the kidneys.

Stop and get medical help the same day for severe stomach pain that does not ease, particularly pain that goes through to your back, a sign of pancreatitis, or for swelling of the face, lips or throat or difficulty breathing. Tell your prescriber promptly about pain under the right ribs, yellowing of the eyes or pale stools, which can point to gallstones; a change in your vision if you have diabetes; or a neck lump, trouble swallowing or a lasting hoarse voice.

Who Should Not Use Tirzide

Anyone who has had, or whose family has had, medullary thyroid carcinoma, and anyone with multiple endocrine neoplasia type 2. That is the precise thyroid warning; a general history of thyroid disease is something to discuss rather than an automatic bar.

It is also not for anyone allergic to tirzepatide, anyone with type 1 diabetes, anyone under eighteen, or anyone pregnant, planning a pregnancy or breastfeeding. If you are planning a pregnancy, your prescriber will usually stop it beforehand. Mention before starting any past pancreatitis, gallbladder disease, severe stomach conditions such as gastroparesis, kidney disease or diabetic eye disease.
