Mounjaro and the KwikPen
Mounjaro is the original tirzepatide, made by Eli Lilly. This listing is the Mounjaro KwikPen: one multi-dose pen that holds four fixed weekly doses of 0.6ml each, which is why it is sold as one pen for one month. Pen needles are not included in the pack, and a new one is needed for every dose; order a box of pen needles with your first pen if you do not already have them.
How tirzepatide works. It is a dual agonist, acting on the receptors for two gut hormones, GIP and GLP-1. The result is more insulin when blood sugar is high, less glucagon, a stomach that empties more slowly and less hunger. Semaglutide, the medicine in Ozempic, acts on GLP-1 only; the two are not interchangeable, and a milligram of one says nothing about a milligram of the other.
What it is licensed for. In the United Kingdom and the European Union, Mounjaro is licensed to help control blood sugar in adults with type 2 diabetes, and for weight management in adults with a body mass index of thirty or more, or twenty-seven or more with a weight-related condition, alongside diet and activity. Your prescriber decides which of these, if either, applies to you.
The effect lasts while treatment lasts. In a trial where people who had lost weight on tirzepatide were switched to a dummy injection, most regained much of that weight within a year. A prescription for weight is planned as long-term, with an agreed plan for stopping.
Mounjaro 2.5mg: the First Pen
Every course of Mounjaro starts with this pen: 2.5mg once weekly for four weeks, whether it is prescribed for diabetes or for weight, and even after another GLP-1 medicine. The full sequence is:
- 2.5mg weekly for four weeks: the starting dose, one pen, not a treatment dose
- 5mg weekly: the first treatment dose, and for many people the lasting one
- 7.5mg, 10mg, 12.5mg, 15mg: only if more effect is needed, each step no sooner than every four weeks
- 15mg: the maximum weekly dose
One KwikPen is one four-week step. Each pen gives exactly four weekly doses, so one pen of each strength covers the minimum time at that step. Your prescriber moves you on, or keeps you where you are, when the pen is finished.
2.5mg is a starting dose, not a treatment dose. It gives the gut time to adjust, and starting higher is the commonest reason for a fortnight of sickness and an early stop. Conveniently, one 2.5mg KwikPen is exactly the four-week starting step.
During that month, mild nausea, a smaller appetite and early fullness are usual and tend to fade. Blood sugar and weight often change little yet; that is expected, and the medicine is judged from 5mg.
When the pen is finished, your prescriber will usually move you to Mounjaro 5mg. Buying a second 2.5mg pen to go slowly is not gentler, only less effective. If the side effects are hard, speak to us or your prescriber before the next step rather than stopping.
Storing Your Mounjaro Pen
Before its first use, keep the pen in a refrigerator between two and eight degrees Celsius, in its carton and away from the freezer compartment. It must not be frozen, and a pen that has frozen must be thrown away even after it thaws. Once started, the pen may be kept at room temperature not above thirty degrees, and it is discarded 30 days after first use or after its fourth dose, whichever comes first. Follow the carton in your hand if it says anything different.
A pen that has been too warm or frozen looks completely normal, so how it was kept matters more than how it looks. We hold every GLP-1 medicine in monitored cold storage between two and eight degrees and send it cold in insulated packaging. On arrival, check the parcel is still cold, the solution is clear and colourless to slightly yellow, and the batch number and expiry match on carton and pen. If anything is wrong, do not use it; call us and we will replace it.
Using the Mounjaro KwikPen
- Prime the pen exactly as the instructions for use in the box describe, and attach a new pen needle for every dose. Remove the needle straight after injecting; never store the pen with a needle on
- Turn the dose knob until it stops. The dose is fixed; the pen cannot be set to a smaller or larger amount, and it must never be used to give part of a dose
- Inject under the skin of the abdomen, thigh or upper arm on the same day each week, with or without food, choosing a different spot each time. The day can be changed if two doses are at least three days apart
- If you miss a dose, take it within four days of the usual day; after four days, skip it and continue on your normal day. Never take two doses together
- After four doses the pen is finished, even though a little liquid remains. That residue is not a fifth dose and must never be drawn out with a syringe. Used needles go in a sharps container; dispose of the finished pen as the leaflet directs, or bring it back to us

Mounjaro with Other Medicines
People who use insulin or a sulphonylurea such as gliclazide or glibenclamide usually need that dose reduced when tirzepatide starts, because together they can push blood sugar too low. That adjustment is for your prescriber. Tirzepatide is not a substitute for insulin.
The contraceptive pill can be less reliable for four weeks after starting Mounjaro and for four weeks after each increase, because tirzepatide slows stomach emptying and changes how tablets are absorbed. Add a barrier method, or use a non-oral method, in those weeks. Tell your prescriber about all your other medicines, and tell any doctor or dentist before an anaesthetic or sedation.
Mounjaro Side Effects, and When to Act
The usual side effects are digestive: nausea, vomiting, diarrhoea, constipation, indigestion and a smaller appetite, strongest in the first weeks and after each increase, then easing for most people. Smaller, less fatty meals help, and so does stopping at the first sense of fullness. With vomiting or diarrhoea, keep drinking, because dehydration is what harms the kidneys.
Seek medical help the same day for severe abdominal pain that will not settle, particularly pain through to your back, which may be pancreatitis, and for swelling of the face or throat or difficulty breathing. Report promptly pain under the right ribs with yellow eyes or pale stools, which may be gallstones; a change in your vision if you have diabetes; and a neck lump, difficulty swallowing or a hoarseness that persists.
Who Should Not Use Mounjaro
Anyone with a personal or family history of medullary thyroid carcinoma, or with multiple endocrine neoplasia type 2. That is the precise thyroid contraindication; other thyroid conditions are something to discuss with your prescriber.
It is also not for anyone with type 1 diabetes, anyone under eighteen, anyone allergic to tirzepatide, or anyone pregnant, planning pregnancy or breastfeeding; a planned pregnancy usually means stopping beforehand. Mention first any history of pancreatitis, gallbladder disease, severe stomach problems such as gastroparesis, kidney disease or diabetic eye disease.
