Gemcal Capsules
Gemcal capsules are used for the management of low blood calcium and related bone problems under a doctor’s supervision, and they contain calcitriol, which is a prescription-only medicine. We would rather tell you that at the top than let you reach the checkout and find out there.
Calcitriol is often confused with ordinary vitamin D, understandably, but the two are different things. Vitamin D3 is a nutrient your skin makes in sunlight and your body activates in stages. Calcitriol is the finished, activated hormone at the end of that chain, supplied ready-made, and it acts powerfully on how much calcium your gut absorbs. That is why it needs a prescriber. If you do not have a prescription, our doctor can see you and decide whether to issue one.
Gemcal Capsules: Why a Prescription Is Required

What Is in Gemcal Capsules
Three ingredients, and they sit in two quite different categories.
Calcitriol 0.25 micrograms is the medicine. It is the biologically active form of vitamin D, and the strength is the clearest signal of what it is. An ordinary vitamin D3 capsule carries 400 to 1,000 international units, which is 10 to 25 micrograms. A quarter of one microgram would be a trivial amount of vitamin D3 and nobody would sell it. As calcitriol, 0.25 micrograms is the standard starting dose of a potent prescription medicine. The number tells you which molecule you are holding.
Calcium carbonate 500mg is an ordinary calcium salt, the kind sold freely over the counter, and it is here because calcitriol works by helping you absorb calcium. The two are given together on purpose.
Zinc 7.5mg contributes to the maintenance of normal bones, and that is an authorised European health claim. It is the one ingredient in the capsule that is purely nutritional.
The manufacturer is Alkem Laboratories. We have printed the strengths above because they are consistent across every source we checked, but we will still read the panel on the pack to you before dispensing, and your prescription governs the dose regardless.
What Calcitriol Is For
Calcitriol is prescribed where the body cannot make its own active vitamin D, or cannot keep blood calcium where it should be. The established uses are the management of low blood calcium in people on kidney dialysis, hypoparathyroidism, and the bone changes that come with long-term kidney disease.
Those are specific clinical situations diagnosed by blood tests, not something anyone identifies in themselves. This is the heart of the matter: calcitriol is not a stronger version of a vitamin D supplement for someone who feels their bones could use help. It is a different medicine for a different problem, and taking it without that problem can raise blood calcium to levels that cause harm.
The Safety Points That Come With Calcitriol
These are the conditions under which the medicine is used safely. They are not a disclaimer; they are the reason a prescriber is involved.
- Blood calcium must be monitored. Anyone taking calcitriol needs their serum calcium checked regularly, because the whole action of the medicine is to raise it. This is the single reason it cannot be a self-service product.
- Too much raises blood calcium too far. Early signs include thirst, passing more urine than usual, nausea, constipation, weakness and confusion. Anyone taking it who notices these should contact their doctor rather than wait.
- Kidney function matters. The medicine is most often used in kidney disease, and the dose is set against kidney function and blood results rather than against body weight or age.
- Other calcium and vitamin D must be counted. Taking calcitriol alongside a calcium supplement, a multivitamin or an antacid without telling the prescriber is one of the commonest ways people get into trouble with it.
- In pregnancy and breastfeeding, only on a doctor’s instruction. This matters here, because the current page suggests the product suits pregnant women without saying that.
There are interactions worth knowing too. Calcium reduces the absorption of thyroid medicine such as levothyroxine, of iron, and of the tetracycline and fluoroquinolone antibiotics, so those need spacing by two to four hours. Digoxin needs particular care, because a raised blood calcium level changes how the heart responds to it.
Who Should Not Take Gemcal Capsules Without Advice
Anyone whose blood calcium is already high, anyone with a history of kidney stones, and anyone taking another vitamin D preparation should not start this without medical advice. Nor should anyone take it simply because they want stronger bones; for that purpose an ordinary calcium and vitamin D3 supplement is the appropriate product, sold freely, and we stock several.
If what you actually want is a general calcium supplement, that is a straightforward conversation to have with us. Our pharmacist will point you at the right one over the counter, and there is no prescription involved in any of them.
How to Get a Prescription From Us
If you already hold a valid prescription, send us a clear photograph of it when you place the order, or bring it to the counter. Our pharmacist checks it, confirms the dose against the pack, and dispenses.
If you do not hold one, ask to consult our doctor. HealthAlone is pharmacist-led and we have a doctor available, and the consultation is a real one: why you think you need it, what your kidneys are doing, whether your blood calcium has been measured, and what else you are taking. Quite often the honest answer will be that calcitriol is the wrong medicine for you and an ordinary over-the-counter calcium would serve you better. We would rather give you that answer than a parcel.
What we will not do is dispense a prescription item without a prescription, or without our own doctor issuing one. That is not a policy we bend for a sale.
Storing It
Keep the pack in a cool, dry place out of direct sunlight, below 25 degrees, and well out of reach of children. Soft gelatin capsules are sensitive to heat and humidity, so keep them away from the kitchen and the bathroom. Leave them in the blister until you take them, check the expiry date on the carton, and return anything unused or out of date to a pharmacy rather than putting it in the bin.

Across Africa, ask us before you order. Prescription medicines cross borders under rules that differ country by country, and a parcel that would reach one capital will be held at another. We have sent orders to Uganda, Tanzania, Somaliland and as far as Malawi, and we will look into Rwanda, Burundi, South Sudan, Ethiopia, Zambia, Zimbabwe, Botswana, Mozambique, Nigeria, Ghana and South Africa for you. What we need first is your own country’s import position on a calcitriol-containing medicine, and we will tell you plainly if the answer is no.
Living abroad? You can order for family here in Kenya, and the prescription still applies. Kenyans in the UK, the United States, Canada, South Africa and the Gulf order from us regularly for parents and relatives at home. For this pack we need a prescription written for the person who is going to take it rather than for you, and our doctor can consult with them directly if they do not have one. Give us their name, their phone number and the estate or town they live in, and we will take it from there.
Why Buy Your Gemcal Capsules from HealthAlone Kenya?
HealthAlone is a pharmacist-led health ecosystem licensed by the Pharmacy and Poisons Board, and our stock is original, sealed and correctly stored. Kahawa Sukari is a real counter with real pharmacists behind it.
On a medicine like this one that matters more than it does on a jar of vitamin C. Our pharmacists will read the panel with you, check what other calcium and vitamin D you are already taking, set out the timings against your thyroid tablet, your iron or your antibiotic, ask when your blood calcium was last measured, and tell you honestly when an ordinary over-the-counter calcium would do the job you actually want done.
It is one more step in our vision: building Africa’s most trusted health ecosystem, accessible to everyone, everywhere.
Frequently Asked Questions
Do I need a prescription for Gemcal capsules?
Yes. The capsule contains calcitriol, which is a prescription-only medicine, so we cannot dispense it against anything less. Send us a photograph of a valid prescription when you order, or ask to consult our doctor and we will arrange it.
Is calcitriol the same as vitamin D3?
No, and this is the most important thing on the page. Vitamin D3 is a nutrient your body activates in stages. Calcitriol is the finished active hormone, supplied ready-made, and it acts far more powerfully on blood calcium. The strength gives it away: 0.25 micrograms is a standard calcitriol dose and would be a negligible amount of vitamin D3.
What are Gemcal capsules used for?
They are used under medical supervision for the management of low blood calcium and the bone changes associated with long-term kidney disease and hypoparathyroidism. These are conditions diagnosed on blood tests rather than by how you feel.
What is the composition of Gemcal capsules?
Calcitriol 0.25 micrograms, calcium carbonate 500mg and zinc 7.5mg, made by Alkem Laboratories. Call us and we will read the panel off the pack before you order.
Can I take it just to strengthen my bones?
Speak to a doctor first, because for general bone support an ordinary calcium and vitamin D3 supplement is the appropriate product and is sold freely. Our pharmacist will show you the options on the shelf.
What are the main side effects?
The significant one is a rise in blood calcium. Early signs include thirst, passing more urine than usual, nausea, constipation, weakness or confusion, and anyone noticing them should contact their doctor. Milder effects reported include headache and indigestion.
Will I need blood tests?
Your doctor will usually check your blood calcium regularly while you take it. That monitoring is a normal part of using the medicine properly, and it is the main reason it needs a prescriber.
Can I take it in pregnancy or while breastfeeding?
Only if your doctor has prescribed it for you specifically. This is not a product to start on your own during pregnancy, whatever a general calcium supplement might be.
Can I take it with my thyroid tablet, iron or antibiotics?
Leave two to four hours between them. The calcium reduces absorption of levothyroxine, iron, and the tetracycline and fluoroquinolone antibiotics. Tell us everything you take and we will write out a timetable.
Is there an over-the-counter alternative?
Yes, several. Ordinary calcium with vitamin D3 is general sale and for most people wanting bone support it is the right starting point. Ask our pharmacist and they will go through them with you.
