What Fybogel’s Ispaghula Husk Is For
Fybogel gives you ispaghula husk, the bulk-forming fibre people take for constipation and for keeping regular, stirred into a glass of water morning and evening. Each sachet carries 3.5 g of ispaghula husk, in an orange-flavoured granule, sixty sachets to the pack.
This is the gentlest kind of laxative there is and the one most people should try first. Ispaghula husk absorbs water and swells into a soft gel, which makes the stool larger and softer and easier to pass. Nothing is being stimulated and nothing is being forced, which is why it can be taken for months rather than days, and why it suits conditions that a stimulant would make worse.
The One on This Shelf That Suits Pregnancy
Ispaghula husk is used for the relief of constipation in pregnancy, which sets it apart from almost everything else here. Constipation in pregnancy is extremely common, and the stimulant laxatives on this shelf are either restricted in pregnancy or contraindicated outright.
It is also used for keeping the bowel comfortable with haemorrhoids and anal fissure, where a soft stool is the whole treatment, and for people with a colostomy or ileostomy. And it is used in the other direction too, for the loose, unpredictable stool that comes with diverticular disease, irritable bowel syndrome and ulcerative colitis, because adding bulk firms a watery stool as readily as it softens a hard one. If you have any of those, please have our doctor set it up with you. Where a bulk former alone is not enough, Movicol sachets draw water into the stool instead and can be used alongside it, and psyllium husk capsules are the same family of fibre in a capsule for anyone who travels.
How to Take It, and the Water Rule That Matters Most
One sachet stirred into 150 ml of water, morning and evening, preferably after meals, and drunk as soon as it is mixed rather than left to thicken in the glass.
The water matters more than anything else here. A bulk-forming fibre taken with too little liquid can swell in the throat or gullet and block it, and that is the one genuine hazard with this product. So use a full 150 ml for every sachet, drink more water through the day, and please do not take a dose immediately before going to sleep, because the gut slows overnight. Keep it at least half an hour to an hour away from any other medicine, because the gel can delay absorption of what you have swallowed with it.
How Much Ispaghula Husk for a Child
Children over twelve take one sachet morning and evening, as adults do. Children aged six to twelve take half to one level 5 ml spoonful of the granules morning and evening, depending on age and size, which means a sachet is the wrong measure for them.
Under six years it is not recommended at all. So please ask our pharmacist for the right amount for a child, bring their age and weight, and our doctor is at the pharmacy if the constipation has been going on for a while, because in a child that is worth looking at properly rather than treating from a shelf. For a child who struggles with granules in water, ask us about Lactulac lactulose liquid, which is taken by the spoon and is often the easier form at that age.
Who Should Not Take It
There is a specific list and it is worth reading. Ispaghula husk should be avoided by anyone who has difficulty swallowing or any throat problem, anyone with a narrowing anywhere in the gut, anyone with disease of the gullet, and anyone with a bowel blockage or hard impacted stool.
It is also contraindicated where a bowel habit has suddenly changed and stayed changed for more than two weeks, with undiagnosed bleeding from the back passage, and where a laxative has already been taken and nothing happened. Each of those is a reason to see our doctor rather than add fibre. Anyone frail or elderly should take it under medical supervision, and so should anyone with diabetes. Please stop and come and see us if you get abdominal pain.
Two Things to Check Against Your Prescription
Ispaghula husk slows the absorption of some medicines rather than stopping it, which is manageable once you know. It can delay minerals, vitamin B12, digoxin-type heart medicines, warfarin-type blood thinners, carbamazepine and lithium, so keep the half-hour to one-hour gap and mention it at your next clinic visit.
Two need more than a gap. Thyroid hormone tablets and ispaghula together want medical supervision, and so does use in diabetes. And it should not be taken alongside medicines that slow the bowel down, such as opioid painkillers, because bulk sitting in a bowel that has stopped moving is the wrong combination. Bring us your list and our pharmacist will go through it in two minutes.
Is Fybogel’s Ispaghula Husk Right for You?
Please talk to our pharmacist or our doctor before you start if any of these apply to you.
- Any difficulty swallowing, or a throat or gullet problem. This is the firmest one on the list, because of the swelling. Ask us for a different kind of laxative instead.
- A bowel habit that changed suddenly and has stayed changed for more than two weeks, or bleeding from the back passage. Please see our doctor first. Those need a cause, not fibre.
- A laxative already taken with no result, or hard impacted stool with pain, nausea or vomiting. See our doctor today rather than adding bulk.
- Diabetes, a thyroid tablet, or an opioid painkiller. Each needs a word with our pharmacist first, and the thyroid one needs supervision.
- Phenylketonuria. Each sachet contains 16 mg of aspartame, so ask us for a product without it.
- Frail or elderly, or treating a child under six. Both want our doctor to set it up.

Fybogel Orange: Your Questions Answered
What is in Fybogel?
Ispaghula husk, 3.5 g in each sachet, in orange-flavoured granules. It is a bulk-forming fibre, so it absorbs water and swells into a soft gel rather than stimulating the bowel.
How do I take it?
Stir one sachet into 150 ml of water, morning and evening, preferably after meals, and drink it as soon as it is mixed rather than letting it thicken.
Why does the amount of water matter so much?
Because taken with too little liquid a bulk-forming fibre can swell in the throat or gullet and block it. Use a full 150 ml for every sachet and keep drinking water through the day.
Can I take it just before bed?
Please do not. The gut slows overnight, so the last dose of the day should be taken earlier in the evening.
How long does it take to work?
Usually within twelve to twenty-four hours, with the full effect over two to three days. If constipation has not settled after three days, come and talk to us.
Can I take it every day, long term?
Yes, and that is the point of a bulk-forming fibre. Unlike a stimulant it does not carry a five-day limit or a dependence risk. Build up slowly and keep the water up.
Can I take it in pregnancy?
Ispaghula husk is used for the relief of constipation in pregnancy, which makes it the sensible first choice on this shelf. Please tell us you are pregnant when you buy, so our pharmacist can set it up with you.
How much can a child have?
Children over twelve take one sachet morning and evening. Children of six to twelve take half to one level 5 ml spoonful of the granules morning and evening, so the sachet is not the right measure for them. Under six it is not recommended. Ask our pharmacist with the child’s age and weight.
Who should avoid it?
Anyone with difficulty swallowing or a throat problem, a narrowing anywhere in the gut, disease of the gullet, a bowel blockage, or hard impacted stool. Also anyone whose bowel habit changed suddenly and stayed changed for over two weeks, or who has undiagnosed bleeding from the back passage.
Will it affect my other medicines?
It can delay the absorption of minerals, vitamin B12, digoxin-type heart medicines, warfarin-type blood thinners, carbamazepine and lithium, so keep half an hour to an hour between them. Thyroid tablets and diabetes both need medical supervision, and it should not be used with opioid painkillers.
Does it contain aspartame?
Yes, 16 mg in each sachet, which matters if you have phenylketonuria. Ask us and we will find you something without it.
Will it give me wind at first?
Some wind and fullness in the first few days is normal when fibre is added to a diet that had little. Starting with one sachet a day for the first week rather than two makes it much less likely.
When should I see a doctor instead?
Blood in your stool, weight you did not mean to lose, a bowel habit that has changed and stayed changed, new constipation after fifty, or bowel cancer in the family. Our doctor is at the pharmacy for exactly that.
